2.) When did you first notice the activity begining? What happened to first make you notice it?

3.) Has anyone been injured or harmed by the activity? Scratched? Slapped? Do you fear for your or your family's safety and well being? If so, why?

4.) Specifically describe what kinds of activity are being noticed?

5.) Have you spoken to anyone else in regards to the disturbances? A nieghbor? The media?

6.) Do you know any history of the property itself? If so, what?

7.) Are there any deaths or violent events in the past of the property that you are aware of? If so, what and when? Who?

8.) Are there any antiques or objects that someone who was once alive may have had a strong bond or emotional connection with? For example, an hierloom, a wedding dress? A dresser, etc?

9.) Has anyone been engaged in any type of occult activity? Ouija Boards? Seances? Summoning? Dabbling? Magick or meditation?

10.) Are stress levels normal in the house right now or higher than usual? Are any relationships strained or any family member under more stress than usual?

11.) What is the most extraordinary event that has occured in recent memory?
