Please complete this form to grant SSPRS official permission to investigate your property for
paranormal activity, to conduct research on and into the property, including history, previous occupants and interviews with
current residents soley for the purpose documenting the activity in question. Only fill out the portions that pertain to you.
SSPRS will fill out its own section upon the return of the form to SSPRS.
I, ______________________, being the owner of __________________________ do here by grant SSPRS ( Southern
States Paranormal Research Society) permission to investigate my property in an attempt to capture paranormal activity that
I feel that I have witnesses on my property. Property herein refers to__________________________.
This permission form is also a confedentiality agreement between myself, ________________________, and SSPRS
(Southern States Paranormal Research Society) which protrects my information and any information that SSPRS (Southern States
Paranormal Research Society) discuss, uncover through research methods or document while on my property.
In signing this document, either by hand or electronically, do hereby grant permission to SSPRS for an investigation
of my property for paranormal activity, conduct research into the background of the property, gain confedentiality protection,
and release SSPRS from liability from damages caused by any supernatural activity on the premisis during the investigation.
SSPRS however does agree to repair any damages caused by accidents of individual team members.
